Fixed Term for 12 months

There are two part time temporary positions available for Community Care Officers. One position is based with the Assessment Team and one position is based with Occupational Therapy.

CCOs are responsible for ensuring the delivery of appropriate services to safeguard vulnerable adults and their families, according to their assessed needs. This includes undertaking all levels of assessments, creating and implementing care plans and support services, and case management.

CCOs are expected to work independently, under the general direction of the Team Manager, and to provide support and advice to colleagues.

CCOs work in various teams across Adult Assessment and Provision, which include Social Care and Occupational Therapy.


- Experience of working with adults experiencing a range of difficulties relating to learning disability, mental health, substance misuse and physical health problems
- The ability to undertake adult-focused assessments of need within a person centred approach
- To have experience of creating and implementing, individually tailored care plans that demonstrate flexibility and innovative use of resources
- To have knowledge of direct payments/personal/individual budgets

Various jobs available - Shortest Fixed Term post is 5 months, others range from 12 months to 3 years. There are also permanent posts.

Social Workers at Level 1 are required to undertake a full induction and complete a successful probationary period. Level 1 Social Workers would also be required to complete the ASYE within an agreed period. Social Workers at Level 2 are expected to carry a full caseload and to work with minimal supervision. Social Workers at Level 2 are also required to deal with safeguarding. Social Workers at Level 3 should hold BIA or AMHP certification.


1. Social Work degree/diploma and HCPC registration, or pending registration for newly qualifying candidates.

2.Experience of risk assessment, safeguarding investigations and understand the need to keep vulnerable adults safe and ability to take initiative when immediate action is required.

3. Ability to analyse information and identify social care needs and how they might be met, with a good understanding of assessment and person centred planning techniques.

4. Ability to manage and prioritise own work, identifying blocks and problem solving promptly, commitment to continued improvement of own practice and awareness of personal strengths and weaknesses and their impact on others.

A number of social work opportunities have arisen in a number of adult social care teams. We are especially interested in applications from newly qualified social workers, those with an interest in Safeguarding Adults and working with adults with Learning Disabilities.
